AMAÇ: Bu çalışmanın amacı, tiroid nodülü tanısıyla ameliyat endikasyonu konulan hastalara yapılan tiroid nodül ultrason elastografisinin, tiroid kanserinin preoperatif dönemde belirlenmesindeki etkinliğinin araştırılmasıdır.
YÖNTEMLER: Kliniğimize, Eylül 2010 – Ağustos 2011 döneminde başvurarak ameliyat endikasyonu konulmuş 60 hastanın tiroid nodüllerine preoperatif dönemde ultrason (US) elastografi uygulandı. US elastografi sonuçları postoperatif histopatoloji sonuçları ile karşılaştırılarak, tiroid nodüllerine uygulanan US elastografinin duyarlılık, özgüllük, pozitif ve negatif kestirim değerleri belirlendi.
BULGULAR: Histopatoloji sonucu 48’i benign (%80), 12’si malign (%20) olan nodüller için US elastografinin karsinom belirlemedeki duyarlılığı %58,3, özgüllüğü %85,4, pozitif kestirim değeri %50, negatif kestirim değeri %89,1 olarak bulundu.
SONUÇ: Tiroid nodüllerinin preoperatif tanısında US elastografinin duyarlılık ve özgüllük değerleri literatüre göre daha düşük bulunmuştur. Buna rağmen US elastografi, tiroid nodüllerinin preoperatif tanısında etkili olabilecek yeni bir ek tanı yöntemidir. Özellikle folliküler lezyonlarda ve İİAB sonucu belirsiz olan olgularda ayırıcı tanıda yararlı olabilir.

OBJECTIVE: The purpose of this study is to evaluate the efficiency of the thyroid nodule elastography, which scanned the patients who were diagnosed with thyroid nodule and indicated operation, in the determination of thyroid cancer preoperatively.
METHODS: 60 patients, who applied to our clinic between September 2010 and August 2011 with thyroid nodule diagnosis and indicated operation, have been scanned by thyroid nodule elastography in preoperative period. Sensitivity, specificity, positive and negative predictive values of elastography, which were applied to thyroid nodules, have been determined by comparing elastography results with postoperative histopatology results.
RESULTS: For thyroid nodules which 48 of them were benign (80%) and 12 of them malign (20%) according to histopathology results, the sensitivity, specificity, positive and negative predictive values of ultrasoundelastography in carcinoma determination are indicated as such; sensitivity 58.3%, specificity 85.4%, positive prediction 50% and negative prediction 89.1%.
CONCLUSION: Even though sensitivity and specificity values of ultrasoundelastography in preoperative diagnosis of thyroid nodules were determined to be lower in comparison with literature, elastography is a new supplementary diagnosis method in preoperative diagnosis of thyroid nodules. Especially it can be beneficial for follicular lesions and some cases with undetermined FNAB results in differential diagnosis.
